The Crawley Work Well project is all about supporting people who are out of work or at risk of losing their job because of health conditions — with a focus on musculoskeletal (MSK) issues. By combining practical advice and coaching, wellbeing support, and links into local services, Crawley Work Well helps people take small, positive steps towards feeling well, confident, and work-ready.
Work Well has been accepting referrals from FCP’s in South Crawley PCN initially, and welcomed referrals from all GP Practices. The service will now be expanding the referral pathways as below:
- FCP’s will expand to making referrals from all relevant Practices (focussing on flagged urgent referrals where a patient’s employment is at risk)
- GPs from any Practice can refer a patient to the Work Well coach by completing a referral form
